Symptoms

Windows Server 2003 crashes when a surprise removal of a USB host controller occurs. Additionally, you receive the following Stop error message:
Stop 0x00000050 (parameter1, parameter2, parameter3, parameter4)
PAGE_FAULT_IN_NONPAGED_AREA

Cause

This problem occurs because the operating system tries to access the memory that is pointed to by the host controller register when the USB host controller restarts. However, the register becomes invalid if the host controller is removed.

When the host controller is removed, the host controller register values are updated to a 0xffffffff value. However, the operating system does not check for this change, and the operating system tries to initialize the hardware. This behavior crashes the operating system.
