Resolution

To resolve this problem, apply the hotfix rollup package that is described in Microsoft Knowledge Base article 955151.
955151 Description of the ISA Server 2006 hotfix package: July 6, 2008
This hotfix replaces the Usr_pwd.htm file with an updated version. The Usr_pwd.htm file is in the following folders:
  • ISA_Install_Dir\CookieAuthTemplates\ISA\HTML
  • ISA_Install_Dir\CookieAuthTemplates\Exchange\HTML
Note In these paths, the ISA_Install_Dir placeholder represents the location where ISA Server 2006 is installed.

If you have customized the Usr_pwd.htm file, the hotfix does not replace the file. In this situation, you must restore the original Usr_pwd.htm file, apply the hotfix, and then recustomize the Usr_pwd.htm file.
