The Microsoft Firewall service crashes randomly when you use ISA Server 2006 to publish a Web server by enabling forms-based authentication

Symptoms

Consider the following scenario:
  • You use Microsoft Internet Security and Acceleration (ISA) Server 2006 to publish a Web server.
  • You enable forms-based authentication in the Web-publishing rule.
In this scenario, the Microsoft Firewall service crashes randomly. Additionally, the following event is logged in the System log:

Description:
The Microsoft Firewall service terminated unexpectedly. It has done this 1 time(s).

Additionally, the following events are logged in the Application log:

Event Category: None
Event ID: 14057
Date: Date
Time: Time
User: N/A
Computer: ServerName
Description:
The Firewall service stopped because an application filter module C:\Program Files\Microsoft ISA Server\CookieAuthFilter.dll generated an exception code C0000005 in address Address when function CompleteAsyncIO was called. To resolve this error, remove recently installed application filters and restart the service.

Description:
Due to an unexpected error, the service fwsrv stopped responding to all requests. Stop the service or the corresponding process if it does not respond, and then start it again. Check for related error messages.
