Symptoms

When extending the PurchRFQEditLines form, the PurchRfqTable send button calls init() twice thus raising an Infolog error, 

Example warning message: 
"A control with the name already exists on the form"

Resolution

Removed the  extra call to init from the purchrfqformletter.
