MPIO LUN loss on storage controller failover in Windows Server 2012 R2

Symptoms

Assume that you have a Windows Server 2012 R2-based computer connected to a storage device through multiple paths by using Multipath I/O (MPIO). When multiple paths fail over concurrently or in quick succession, the I/O operation may fail, and the computer may lose its connection to the storage device.

Note This issue can occur with any disk that's connected over multiple paths by using MPIO.
