Two cursors appear with multiple monitors in Windows 8 or 8.1

Summary

On a system running Windows 8 or 8.1, using multiple monitors with multiple display adapters, the display can be configured in such a way that two cursor images can be seen on one desktop.

If you are using two display adapters, and one supports the “hardware cursor” model while the other supports only the “software cursor” model, this issue will occur because Windows expects both adapters to use the same cursor model.
