Symptoms

On Windows Server 2008 R2, when private network for storage is configured to use both IPv4 and IPv6, the Microsoft iSCSI Initiator never reconnects after failing and re-establishing a path to storage when Multiple Connections per Session (MCS) is configured on the Multiple Connected Sessions property box. The status always shows Reconnecting  This also happens when setting Favorite Targets and rebooting, connection is never made to iSCSI storage. There is no accessing the iSCSI disks because of this issue.
